Hi Kevin, I'd keep it simple and use Word or Pages (even thought I don't know for sure that both allow PDF inserts but I assume they both would considering its 2011).
The new Word has a fullscreen (distraction free ;-) writing mode. I assume from what you have said in other posts that you will let the publisher fool around with the final formatting and conversion to HTML and eBook. That said, I'm also hoping one day to write some books with Scrivener (or at least that's why I bought it) and our boys use StoryMill (simpler that Scrivener and more targeted at fiction?).
