Gmail/Google does support IMAP IDLE, so on macOS it *should* be working automatically. But, bugs in the implementation mean that sometimes it stops idling, so you can't rely on that. If it doesn't work reliably, check the check mail interval is set to every minute in Mail's settings; it might be delayed, but only by a minute max. iOS doesn't support IMAP IDLE because it's stupidly conservative, so yes, just swipe to make it work. Google stopped supporting ActiveSync a while back, and that's the only protocol Apple (officially) support for push for use by Microsoft Exchange servers, and so requiring you use the Gmail app, or Outlook, amongst others, for app-delivered push.
Privacy? Well, if you're using Google, you may as well just give up. But, although I do think Apple's privacy messaging is somewhat lop-sided and hypocritical, yes, I do think there's a difference: Apple does appreciate where the red lines are. As I said, Outlook will require you hand over your email from another unrelated party to them. You have to be happy about that. The fact that Apple can effectively advance privacy as a shield by conveniently locking you in to their services for certain iCloud-only functionality like push is irritating beyond words, but the issue is fundamentally one of policy to withhold interoperability information and not technical possibility; Apple would only have to document the IMAP extension it uses and provide a way for servers to obtain the necessary push certificates for free.
