Hi Simon and E.T. I know what's going on. Since the first release of El 
Capitan, and even in the betas, there's been a bug when after you use Safari, 
the VO navigation rotor for some reason gets set to Window Spots. I've written 
to Apple to fix this bug multiple times but still they haven't done anything 
about it as you can see. While we wait for the bug to be fixed, set your 
navigation rotor to navigation by pressing both left and up arrow until you 
hear Voiceover say navigation. And like I said, if you use Safari and after 
you're finished with it, do this command and up and down arrows should work 
again.
