Robert, although the problem is annoying, this approach you described might at least be a usable work-around. I will try it the next time Voice Over abandons me. Thank you very much.
To all of you who wrote, thank you. Sometimes, the only way to recover has been to just hit the Power key, wait a second or so and then just press Enter. Not an elegant solution, but at least it has worked since "Shut Down" is the default selection in this dialogue.. I will definitely post a message to the Accessibility Group, apprising them of something they probably already know by now.
