Hi,

This sounds exactly like the mute switch is active like others have mentioned.  
On the left side of all iPhones, just above the volume up button, there is a 
physical slider switch.  Slide the switch towards the face of the iPhone to 
have all sounds for Ring-tones, Messages and other alerts active.  Slide the 
switch towards the back of the iPhone to mute these alert sounds.  If vibration 
is turned on, the vibration alerts will still occur but those ring-tones, 
Message alerts and other alert sounds will remain silent.  It's sort of like 
"Manner Mode" on older flip-phones.  When VoiceOver is turned on, all regular 
speech will happen including VoiceOver announcing incoming calls but the sound 
effects will be silent.  Also, when VO is on, when this slider switch is slid 
to the "Mute" position, VoiceOver will announce "Muted".  If you have an iPhone 
cover like the OtterBox, there is a flap that you must pull back in order to 
access this slider switch.
