What is your input?  Is this running through a scalar?

Possibly, especially since a 1.0 USB port isn't quite fast enough for
the device.  Does the output of dmesg have anything in it about
resetting a USB bus?  Have you tried a different USB port or just
unplugging the mouse temporarily?
