Title: Medication Reconciliation at Discharge
I'm just wondering what everyone is using for Medication Reconciliation at Discharge? Especially when it comes to the discharge instructions...we would like to be able to pull in both the home meds that the nurse enters and any ordered meds from Pharmacy...then allow the nurse to edit whether they've been discontinued or not at discharge....does anyone do this? Is it possible? If not what is your hospital's process. We are a Magic 5.4 hospital.
