In it's simple form I often use a 1" piece of water pipe that I've cut up
over time. The good thing about water pipe is that it fits nicely over your
expensive 1/2" drive Sidchrome ratchet and you don't have to belt the
ratchet with a Mexican screwdriver (which they tell me doesn't do anything
good to them) to get a stubborn nut to shift - only small problem that will
get you once and a while is you'll break the damn bolt off but that's a
different problem altogether and we've covered it recently.

Well, since you asked, yes, and some socket sets ie Sidchrome already
have one in them.  It's a 1/2" socket drive which slides up and down a
cylindrical bar.  However, it's only really useful where you can't fit a
ratchet- unless you slide a nice long bit of pipe over the end, which
increases the torque on the nut, because........

Torque = Force * Distance

Toms' Crap Diagram (tm): increase the distance to increase the torque on
the nut
