_Loops_ are the big problem. That, and the assumption that _any_ conductor is even remotely perfect. Especially dirt. These factors are negligible in 'ideal' circumstances, and crucial/deadly in non-ideal circumstances. Codes are the way they are for very good reasons.
Lightning is about as non-ideal as it gets. (Something to do with millions of volts, backed by millions of amps...
