Assuming the CORRECT belt is used, the problem is likely that it is not  
aligned properly, meaning that there is an ANGLE over one or more of the  
pulleys 
that the belt is crossing (perhaps because a pulley bearing has worn),  and 
that angle is allowing the belt slippage.
However, before doing anything else I'd verify with a proper diagram  that 
the belt was put on correctly (that the belt is indeed going over or  through 
the pulleys in the correct fashion as per the diagram).
