I've just committed a patch adding dry/wet knobs. I improved
(hopefully) thread safety today as well and added LADSPA menu because
the LADSPA window is hidden by default now. There's now a wiki page
with some tips too:

You can assign MIDI controls to the dry/wet knobs by using
"[LADSPA],DryWet0", "[LADSPA],DryWet1" etc. in the MIDI mapping.
