Well, that is not what I am talking about.  I have a big box wrench (?) that
is offset enough to drop around the nut.  I then hold the insert with a
allen socket and sping the nut loose.

You can use a deep offset wrench.  Than works just fine.

I had to make one this weekend.   Took about 15 minutes with a grinder 
(because I messed up twice) and a $3 deep 9/16" socket I bought from 
discount auto.  Not  sure where you can buy one.
