Difficult situation. If it helps, I have succesfully compiled a MonoTouch app 
with my client's certificates. My MT license is Personal/Professional, the 
client did not have an MT license, only iOS developer enrollment. The app is 
not yet ready for the app store, but I have compiled for ad-hoc and they 
install it on their devices without problems.

I do not know what happens in the case of Enterprise distribution. My guess is 
that you must have the MT license in order to compile the appropriate 
certificates and your client must be enrolled for enterprise distribution with 
Apple. Don't count on this though...
