Dear Michael, et al: I don't know how they did it in the old days (it was probably the same), but nowadays, you hire a still photographer who slinks around the set taking pictures of whatever he/she feels like. Sometimes they get a shot from the same angle as the camera (and it looks like a frame from the film), while other times they get different angles (which usually aren't lit properly, as the lighting is set to the angle the camera is recording). They might set up posed shots (for the poster/stills/etc...), sneak around and take candid shots, do technical shots to record placements, etc... And, as an interesting side-note, since most movies record dialogue, the still photographer has to have a sound-proof case for the camera...
