Take a look at Nancy Akhavan's Accelerated Vocabulary. It is very  
sequential, involves students in the activities and actually provides  
the overhead transparencies in an envelope in the back of the book. Of  
all the vocabulary programs/books that I've looked at over the years, I  
believe this is the most practical and sound. Amazon gives a way better  
description of it than I can here. It goes month by month and builds.

The problem with vocabulary is you can spend a lot of time "teaching"  
words that some kids already know or that kids only understand a  
certain level. Akhavan's book works.
