Debbie - Have you looked at any of Benita Blachman's work? She's done a lot with Phonemic awareness as well as phonics instruction. Her book Rode to the Code has been out for a bit now and focuses on Phonemic Awareness and maybe some early phonics. I believe it's designed for Kidergarten, so maybe it could help out you little one. For the older ones, her new book Road to Reading just came out in January. It is a 6 level phonics-based program designed for 1:1 and small group instruction, but can be done in a whole classroom as well. She lays out a 5 step lesson plan and provides resources for every step at every level. It's very easy to mold to individual students. It was built out of her research with 2nd and 3rd graders, yet I used it with a 7th grader this spring and found it very usable, but I did supplement it A LOT because many of the words she includes were sight words for him. (He's a lot like the 4th graders you describe - He had an extensive bank of sight words but couldn't figure out any new word.)
