I'm not sure my troubles with the compact phrase table format were
related to placeholders. I experienced segments disappearing from STDOUT
when I enabled the *-print-alignment-info* and -*report*-*segmentation*
options
(https://www.mail-archive.com/moses-support%40mit.edu/msg11910.html on
3/15/2015). During the same timeframe, there were other reports of
compact format stability problems, and the effort to update that code to
run on Windows appeared overwhelming. So, we took the easy path simply
reverted to the legacy binarized file formats. I never finished
troubleshooting the output problems.
Re placeholders, initially they looked like a good thing when they were
introduced in R2. We found that we can achieve the same results with
pre- and post-processing in other ways. So, I've not kept up with how
the native Moses placeholders work anymore.

I'm not using mosesserver yet, just moses.
One previous report was here:
https://www.mail-archive.com/moses-support%40mit.edu/msg12355.html
But I'm not sure if this was with moses 2 or 3. There was a later report
from Tom Hoar with similar information.
