There are hundreds of people that have access to the Mozilla code servers. Fixes are checked in by those people and then we have a machine in Austin that builds the code at scheduled times in the day. It then uploads them to mozilla.org.
We specifically picked our build times in an attempt to help stability (8AM PST is when the tree closes for the verification builds, and 4PM PST is usually good enough to catch any fixes that fixed the verification builds) Milestone builds are done by hand by me to ensure that they have any OS/2 specific fixes that have not been checked into the trunk yet.
