This is usually down to problems with the CD-ROM drive, which will usually
return data from a position on the CD slightly offset from the requested
position. Most CD rippers have an option to compensate for this, but EAC
(http://www.exactaudiocopy.de) is the only one I know of that offers a
reasonably easy way to calculate it - based on a database of particular
pressings of fairly common CDs. If you have a CD recorder, it is possible to
compensate for the offsets in the reader and writer, and therefore quite
possible to produce exact duplicates of audio CDs. Plextor drives seem to be
preferred for good quality ripping/copying.
