John, I cannot answer all the questions, but I use the portable version, if you used that then installation on multiple machines, re-formatting your drive would also not then be an issue.
As for the connection to the internet, and activation expiry, I don't think once installed the licence is checked i.e. I don't think MLO connects to a licence server periodically, but I am not certain of that. Someone else more familiar should be able to answer that for you. But I use MLO Windows portable mostly behind firewalls, which would prevent access to a licence server, so I would have thought I'd have hit the problem by now.... I would send your questions to the pre-sales contact address to get the definitive answer...
