Would it be possible to post all config files involved or email them to
me? I'm still having some trouble getting this to work. I can run
mplayer -aw hwac3 and my amp reports that it's getting Dolby Digital,
but I don't get any sound. This seems like a mixer problem maybe
rather than in input problem? Xine never seems to work, and I've tried
all of the suggestions in this thread as far as I can tell. Can
someone post a comprehensive list of what needs to be looked at? i.e.
asound.state, .asoundrc, xine config, etc.
Thanks so much for the input you have all provided so far. One email
with all of this info would benefit others in the future I'm sure.

I had to add

I made all my changes through the GUI so have never actually looked in
the config file. I did have to set that option to Pass Through in the
gui panel though so it makes sense.
Is it possible to attenuate the AC3 digital signal? Previously, when
Xine was configured to output PCM through the same digital coaxial
cable, it was approximately 8-12 dB louder than Mplayer with PCM.
Now with AC3 working, the volume is the same. There was a DSP gain
plugin for the emu10k1 driver back in the days of OSS. I never had
AC3 working back then so I'm not sure if it could attenuate a AC3
signal. Is attenuating possible with ALSA and SBLive!? If so, how
can I do it?
Don't know. I don't watch DVDs that often so haven't really noticed
the volume diff. Most of my mythtv usage is watching live/recorded tv
or mythmusic which all appear to have similar output volume levels. I
never use any of the myth/xine volume controls anyway.
Do you still need any config files?
