A doddle.  I'm using a surf squirrel in exactly the way you intend to
do.  The only problem I had was AmTerm didn't like the older drivers. 
You need surfsquirrel.library version 40.45 and squirrelserial.device
version 37.565 (also squirrelscsi.device V37.1823) which are available
from HiSoft's website if you don't have them already.

All you do is set up your Modem->Device settings to
squirrelserial.device unit 0 and shove the baud up to 115200 (or as
high as the modem will go) and it works. Very little cpu load too. :-)

Hmm.  You can't plug in two pcmcia cards at once... and don't believe
it when they say they're "hot" unpluggable - there may be no
electrical damage but it often crashes the computer.  Your only hope
is to take the lid off the actual cd drive box and try to determine if
its scsi or ide.  If its scsi you can house it in an external case and
connect to the squirrel. If ide you might get it working off an atapi
adaptor on the internal port.  Tell us what make and model it is and
someone on this mailing list might know which method to take.
