A foot of flexible vinyl tubing. :p No, seriously. I don't use any fancy bleeder systems, I just pump it through with the master cylinders.
My usual interval, given how wet it is down here, is once a year. Brake fluid is CHEAP, and one bottle does both clutch and front brakes if you stay on top of it. If your fluid is that old, you'll probably want to pump the whole bottle through the system.
