As far as audio, nothing tops a feed from the soundboard, I have done
what you are wanting.
Plan on using the venue soundboard.
 Maybe have a mike on the crowd to record an audience track if you
want (fed to your computer directly, dont plan on having the engineer
mix a crowd mike into the board just for you)


For video you have options:
DV cam is very nice option, you have it to use separate from the
streaming setup if you want.

a cheaper option, since you are planing on streaming
get a capture card/dongle (any wintv product) and a camera from
anywhere, even Harborfrieght has a good color cam (it has a mic, but I
am not saying use it)

A super simple solution is to use a video capture card and a composite
camera and install zoneminder to stream it
quick, dirty, cheap 1,2,3
Once you have it going with ZM you can workout wrinkles from a working
system and decide where you need to spend money and/or time
