I applied the SP without stopping IIS (hadn't heard about this yet) and now OWA does not work on my Exchange/IIS server. Is this most likely related? OWA works within the network, behind the firewall, but not from outside.

Stop the IIS service before applying the SP. Also, there is a TREMENDOUS DLL clean up operation that can rack the processor util around 100% for quite some time after the reboot. It resolves itself.
