If you are not logging web events (shame on you :-) goto IIS manager and enable it from the general tab of the default website.
Check and see if you get anomalies. What might have happened, is your security settings might have been reset. internal people might be passing their authenticated credentials along, but external is not..... (just a thought out of left field)
Are you using host headers? Are they still the way you originally set them up (if you're using them) ?

If you have used the ISS Lockdown tool with or without URL Scan. Stop the IIS service before applying the SP.
Also, there is a TREMENDOUS DLL clean up operation that can rack the processor util around 100% for quite some time after the reboot. It resolves itself. DON"T try to debug it by doing stuff. let it run it's course.
