I'd suggest Spotlight on AD if monitoring AD and Exchange specific functions like replication & synchronization. Spotlight can also monitor your Blackberry environment, if you have one. While MOM's the Cadillac of monitoring it does come with a hefty price tag so I guess the main questions for me would revolve around the size of your environment, number of servers, DC's, exchange servers, etc.
