We'll provide a form which generates the license with your particulars and an expiry date stamped into it. Bear in mind that you will generally use this only for the commercial side of your project, you'll have GPL or whatever on the Open Source side. Any commercial license you have on your product will have to answer all your questions, so this is no extra.
