Specifically, your product has one problem: It does not clearly identify what parts of the work are Open Game Content.
Parts of the program are a deriviative work of the System Reference Document material. Those parts need to be made Open Game Content. Then they need to be Clearly Identified as such. And then you need to alter the OGL you're using to update the COPYRIGHT NOTICE section of the license.
