I confess I've only been skimming this, so please forgive me if this is 
redundant or otherwise invalid, but have you considered:

Creature Name: Greater Demonic Phase Flumph
Creature Full Name: Greater Demonic Phase Flumph, Created by Lizard

In the designation of OGL:

blah blah all creature stats are Open Gaming Content except for 
"Creature Name" and "Creature Full Name" which are Product Identity blah 
blah

Special License for use of Product Identity:THe right to use the 
"Creature Name" product identity is granted, provided the "Creature Full 
Name" for each creature whose "Creature Name" is used appears in the 
text at some suitable location. (Obviously, more legalese is needed)

Thus, to use the PI "Greater Demonic Phase Flumph", the PI "Greater 
Demonic Phase Flumph, Created by Lizard" must appear *somewhere*. This 
allows them to use the 'short name' throughout the text, while the 'long 
  name' appears only once, probably in the front or back of the book. 
Reproduction of the sub-license of the PI must also be mandated, 
allowing anyone deriving from the derived product the same rights and 
responsibilities.

The "Names are PI" has already been tested with CCI. The special 
sublicense to freely use PI was tested with R&R. This just goes one step 
further.

Again, if this has been discussed, I apologize for wasting everyone's time.
