I would go with #2, despite my earlier expressed
admiration for your inventiveness in coming up with
#1.  Here's why.

I don't really see the preference for S.15 citation as
being an incentive or disincentive for reuse of the
monsters.  If a producer wants to reuse a monster, I
think they will no matter which format is chosen.  

By going with #2, however, you greatly increase proper
citation in S.15.  Despite you explicit instructions
included in your original and revised #1, someone at
some point would probably become confused and make a
mistake in the S.15 attribution.

A tangential issue is that S.15 under the revised #1
could become very lengthy with reuse of just a
half-dozen or so monsters.  Again, I don't think it
would keep a producer from reusing a monster, but if
their OGL page now takes up 2 pages in the product
instead of 1, it could cause some irritation at the
least.  I could see the situation, however, where a
producer decided to cut one monster from the final
manuscript (if the monster was not pivotal to the
adventure and a similar creature from the SRD was
available for substitution), just to keep his OGL page
at 1 page length.

In the end, I really hope that anyone who reuses a
monster from the Tome of Horrors gives proper credit
to the original authors.  As I believe Alec suggested,
perhaps a simple "Please include the full credit
citation as listed in each monster's description
somewhere in your publication" would suffice.  It
seems as if every producer so far has been polite and
respective of other producer's contributed OGC,
contacting them before reusing their OGC and giving
thanks in their credits pages, and I would expect that
any producer reusing creatures from the Tome will
gladly credit the original authors as well.

By the way, I love your intention of footnoting the
publication in which each monster originally appeared.
 That will be the first line I read in every
description.
