I've found it doesn't matter how clear you make things, people will find a way to be confused. I get e-mails with alarming frequency like this:

"Hi, I'd like to use some open content from Book X but your designation of OGC is confusing. Can I use feat/spell/item from page Y of Chapter Z."

I will dutifully haul out my book and double check my "confusing" designation. Nine times out of ten, it's something like, "The following text is OGC: Chapter Z." Or "Everything on page Y under Header XXX."

Today, I had a freelancer ask me if he could use a few critters from Tome of Horrors in an upcoming adventure. He said he wasn't sure if they were OGC. This, as Clark points out, from a book that tells you exactly how to cite OGC from it! You really can't win.
