As I suspected, this fixed the problem! Good work!
One reason I wanted to look at the HTML version was to see if the
authors are listed on the title page there too. And they are - BUT -
there is no page 2 equivalent so eliminating them from the title page
would eliminate them altogether. The bottom line is we don't need any
changes for the HTML versions.
