Hi

We use RHEL5 with MPIO into an EqualLogic SAN.

 From what I remember setting up MPIO is that you can configure in the  
SAN how many simultaneous connections are allowed per volume.
Your post mentions another volume working fine with MPIO, but doesn't  
say wether this is something you already have thought about.
