Regarding your question: As far as I know, you can just replace the
existing heightmap image with a black(all values=0) one in order to make 
it flat. Editing the ppm file is best done using an image-editor, though, 
as it is easy to get something wrong when editing it by hand...

The .htmp file is a compiled version of the heightmap. So after you have
edited the heightmap, you have to regenerate the file. It might be 
sufficient to just delete it, or maybe you have to explicitly rebuild it, 
as stated in [1], Chapter 5.

As for creating Collision maps, this is also covered in [1].
