There is a not-so-obvious way of creating an intermediate point without 
having to start over.  

   - Mark it with a long press
   - Press "Directions" and add it as a first or last intermediate 
   destination. 
   - From the directions menu, press the flag icon at the bottom (to the 
   left of the gear).
   - From the "Options" menu, press "Sort door-to-door"

In addition or alternatively, the Options menu allows you to manually 
rearrange the points, as well as reverse the route 

I could imagine setting a starting point and destination, then filling in 
with intermediate destinations until the proposed route goes where you want 
it to.  Then saving the route as a GPX.  Routing on a GPX created that way 
appears to give proper turn-by-turn directions, although,obviously, it also 
routes to each waypoint as well.  

Admittedly, if the goal is to do true turn-by-turn navigation using an 
externally created GPX while allowing for graceful detours from that GPX, 
this is a workaround.
