Hi, I have worked with Quads and with Bilateral hemis. their treatment for
the most part is very different. The Bilateral hemi has cognitive and
perceptual motor problems that are not usually seen by a quad unless there
was a closed head injury involved, A Quad does not have the cognitve
deficiets that a neurological hemi has. Both need attention for
positioning, contracture prevention, muscle facilitation and inhibition to
control tone, more so in the hemi most often. A hemi's progress can really
be inhibited by the above cognitive problems and perceptual problems as far
as safety and judgement, they might be able to walk, but not see anything to
the left, impulsively put themselves in danger and cannot be trusted with
meals, money or medication. Quads without a head injury are much more
reliable with those things but need outside assistance for accomplishing
those tasks, ie adaptive equipment or care aides. Hope this helps .Wanda
