Ron I believe what you are referring to is an approach called NDT or Normal
Development Therapy. This is often wrongly mixed up or used interchangably with the
Bobath concept. Bobath, in my understanding, looks at muscle tone and core stability
ie strength in the trunk. It concentrates on what the underlying muscle tone is
causing in terms of movement. So a person who has difficulty moving their upper limb.
Rather than looking at the "affected limb" and treating that, one looks at the whole
person and the over all posture and tone. So for example if the person has weakness in
the trunk and pelvis, they will not gain enough stability to move their upper limb and
may in fact be using the "unaffected side" to hold themselves up. Working on their
arm may not be effective until correcting the weakness in the trunk.
Personally i think the main benefit of using a Bobath approach is looking at
the person as a whole. From my experience those physios who use the Bobath appraoch
have clients with better return of function in the arm and hand. If a compensatory
appraoch is used then the person's arm tends to become tight as they try to recruit
strength from other parts of the body. This can lead to permanent deformities which
may have been preventable.
I believe that an appraoch is only as good as the level of skill a person has.
I am not Bobath trained and therefore can only use some of the principles and have
been lucky enough to work with OT's and Physios with full training and lots of
experience and I have learned a little bit from them. It takes over a year to do the
full trainingand includes a full week of intense training. It can be expensive. I
believe that we should choose an approach to fit the individual client and not
everyone wants to use their arm or to walk without a limp etc etc. So there are pro's
and cons.
