Wow, that's horrible!  OK, I'm certainly no linguist, but I'm going to
go out on a limb here and say that the first field is your name, the
second is your email address, and the third is a big text box with 11
rows by 45 columns into which you can type whatever you like.  I
looked at the HTML source for that form, and the names for the input
fields and their associated size parameters make me think this way.
The first field name is "nimesi".  I get the word "name" out of that.
How about you?  The second field name is "oitteesi", which gives me no
indication of what it might translate to in Danish or wherever it is
he's from, but considering the three things that form is asking for,
email address would be the next most likely thing.  Both the first and
second fields have a display width of 30.  The first field has a
maximum field length of 255.

And if you don't buy any of the above, well, I can tell you for sure
that the last field is free-form input.  You can try sending the form
twice, once with your name first, then your email address, then again
vice versa.
