When I was looking at the dip switches, for some
reason I was reading mine upside from the way the
picture looked.

This was a different card.

I also had trouble changing the zif on a G3, as I
would reset the cuda switch, and kept zapping the pram
and had the same problem.

After giving up I decided to reset the cuda switch one
more time, without zapping the pram, and it stopped
freezing.

I don't know if it would even be related, but
sometimes anything is worth a try.

This was after rechecking everything including cables
and anything including dimms and cards to make sure
they were fully seated.
