Put in a bootable CD with a conservative older OS (like 8.6 or 9 or 9.1) and
does it still happen? Start with a minimal Extensions set (adding only any
Powerlogic extensions), still happen?

What Hard Disk, how big and how is it set up. Is there spare room on it?
Defragged it lately? Back everything up now in case it is on the way out.
