That is correct, they need to go on all pages.
Lastly, is there no way to cut and paste buttons? (I'm still using Acrobat 4.0 for MAC but would (should) upgrade to get features.)
Acrobat 5 & 6 allow you to copy & paste buttons...You should upgrade!
