My list is somewhat similar to yours, but there are some differences.
How much I carry depends on what and where I'm shooting. For some
situations, I'll carry a large amount of equipment, for others, much
less. But the following is usually in my kit:
lens cleaner
lens cleaning cloth (in a sealed container)
lens blower brush
remote electric shutter release
polarizers, yellow filters, red filters
flashlight
magnifying eyeglasses
pentax spotmeter or Vivitar incident meter (sometimes both)
monopod, mini tripod, or full-size tripod
lens hoods
I generally don't take a memo pad and pens, although I should, since I
sometimes forget exposure data by the time I get home.
