The two exposures look to be hand-held and the framing is slightly different. That could be affecting the flash meter reading. But who knows?

I use the in-camera flash so rarely I haven't spent any time trying to figure out the consistencies/inconsistencies. When I use flash right now, I'm using a non-dedicated Sunpak 383 with a bounce diffuser. I make a test exposure and then tweak the aperture setting until I get the results I want. Once I've dialed in the exposure, it's very consistent.
