I shoot on very uneven ground a lot, and I find the ball head very helpful. Sometimes the terrain forces me to use the monopod at an angle, and the ball head let's me get the camera back near level ... then I just tilt the whole rig for fine tuning. But without the head, sometimes I'd be screwed.

I use the Bogen/Manfrotto 222 Grip Action Ballhead - Supports 5.5 lbs (2.5kg) a pistol grip ball head which works fine for me with my 300mm ƒ2.8 (Tamron). Quick loose and quick set. (Used to be the #3265 - mine is anyway).

I pick the monopod up, release the trigger, point the lens, let go of the trigger, plant the foot, fine tune. It has a built in quick release and comes with one Bogen plate.
