I don't go anywhere with the intention of insulting anyone. However, if I go into a restaurant, of whatever fare or cost, and they offer me a menu. I'll order from the menu. If they ask me how "well" to cook it, I expect them to comply. If they're not going to comply, don't ask. Instead, mark the menu to tell me what to expect. But if you go out of your way to ask, then berate me because you don't like my response, you're an asshole.

It's a two way street. In my experience, it was more often the customers who were assholes than the cooks. Most of the chefs that I worked with wanted to send out food that was edible, and felt that a steak cooked past medium well was a waste of good food.
