FWIW (and your doctors might be different), I've never been charged for a copy 
of ANY information from the doctor.

When my son did a high-school report about his 8-year sequence of surgeries to 
fix a facial injury he incurred when he was 10, he asked the various medical 
centers for copies of his "imaging" and received quite a stack of CDs for no 
extra cost.  The paperwork, etc, is considered "yours".

YMMV, as they say.
