The different subfolders on the SD card is triggered by the mechanisms Dario listed.
Regarding your situation with Lightroom, I'll make a guess.

Maybe it's caused by two factors working together; unchecking the "include subfolders" option and previously pointing the import plugin to begin with the 100PENTX instead of the DCIM parent folder?

Last night I saw the Red Elvises play in Santa Cruz.  It was a very fun
show, and I ended up taking pictures. I loaded them into lightroom last
night, and when I went through them this morning, a whole bunch were
missing.  Going back and looking, Lightroom had only read the ones out
of the 100PENTX directory, not the 101PENTX directory.

First of all, this seems to be a bit of a lightroom bug. However, what
causes the camera to change from storing photos in one subdirectory to
another?  Is it when I hit 500 photos?
