Dan, In addition to Phil's comments check your layer stackup! There's a knowledge base article describing such effects particularly in blind/buried designs if the layer stackup is not in a regular order (i.e. top, mid1, mid4, mid3, mid2, bottom instead of top, mid1, mid2, mid3, mid4, bottom). Don't know if it also applies to to standard through hole technologies.
