I have not heard of too many on dosages higher than mine (not really proud to have the dosage champion title, however).  Most of the time I feel OK, though I have been taking the meds for almost 11 years, so this is my "normal" - - I don't think I am really very good anymore at identifying medication-related problems, though I know I tolerate the meds better than what appears to be the vast majority on the boards.  I have none of the terrible side effects that they have.

In my humble opinion, if there is no urgency (i.e., optic nerve issues, carotid artery interference, etc.), I would slow down the scalpel cowboys, and get many opinions before I lay down on that table.  11 years ago, I nearly had unnecessary surgery when my condition was improperly diagnosed.

Fasting for the prolactin test seems to be an open issue on the boards - - some doctors recommend it, others do not require it, others seem unaware of the issue entirely.  The consensus from the pit. Masses, however, seems to be this:  no matter what you do (fasting or not; time of day; sleep; routine before testing, etc.), do it consistently.  Then your numbers from test-to-test will be comparable.
